Spousal Surcharge: $200 per month ($92.31 per pay period) Waived when Spouse is not enrolled in DFW’s medical plan, is unemployed/self-employed/retired, or does not have medical benefits available through his/her employer.
LiveWell Credit: $75 per month ($34.62 per pay period) will be applied to the medical premiums of employees who earned at least one LiveWell Incentive Apple in FY25. The credit will be automatically applied to employees who were hired or joined the plan after June 30, 2025

On-site clinic operated by Concentra offering a wide range of medical services.
Open to all employees.
No cost; HDHP members may be required to pay a small fee for non-preventive services.

On-site weight management and nutrition program through the LiveWell Health Clinic.
Provides baseline assessment, coaching, monitoring, counseling, wellness plans, and lifestyle guidance.
