- Short-term disability is administered by Unum as part of DFW’s Leave Administration program.
- Eligibility requires six months of employment at the time of initial injury or onset of illness.
- All applications are reviewed and approved by Unum.
- Employees hired before 2011 are paid at 85% of base pay for weeks 13–26 instead of 70–85%.

LTD Notes
- Eligibility requires six months of employment at the time of initial injury or onset of illness.
- You must be unable to work for more than 180 calendar days due to an approved short-term disability or workers’ compensation injury or illness.
- You must be totally disabled as defined by the plan.
- Maximum benefit period: Continues as long as the plan's definition of disabled is met, or until age 65. If the long-term disability benefit begins at age 62 or older, benefits may extend past age 65.
